The NHS reply-all email fail last year involved 500 million emails being sent across the health service's network in just 75 minutes.

A test message sent on 14 November to what an unfortunate "senior associate ICT delivery facilitator" thought was a local distribution list she had created instead went to all 850,000 people with an NHSmail email account. The blank message, sent early in the morning with a subject line that simply read "test", was sent to a distribution list called "CroydonPractices". Around 80 irritated folk promptly hit "reply" to demand they be removed from the list – which was when the meltdown began.

    The number two guy at our company sent out an email to the entire company detailing the plans for reorganization (read: burning the company to the ground and taking C-Suite parachute) and copied every address in the system into the CC line (which meant everyone got it multiple times as department emails and other lists were also included). Well, someone hit reply all and sent an email asking about a department not mentioned. Then a bunch of other people hit reply all to chastise that person for hitting reply all. And then people did the same to chastise them. Over a week later, people were still hitting reply all to chastise those doing so. Sure, most by then were jokes carrying it on as everyone knew they were doomed, but there were quite a few that were sincere. Thankfully, IT took it with humor as the data-deduplication keep the server from croaking under the load.

    So one day I showed someone how to use ye ol windows command 'NET SEND'. Within the hour *bleep*. 'mike you are a girly man'. 300+ computers with that message on it. If you do not get it just right it broadcasts it. Thank god it is disabled by default these days :)
