So, these "hundreds and thousands of smaller internets" use IP addresses outside the RFC1918 and RFC 4193 ranges do they? They either use a globally routeable IP address and are part of the Internet, or they are a private network (internet, or perhaps intranet).
It's the whole "deciding to work together" part that makes it a single, named Internet. For example, what the author is saying is that the USA doesn't exist. Instead, there are millions of people who live together in part of North America.

It's funny because you don't understand American federalism either. The United States of America is a Union of States in North America that decided to work together for the common defense.
